Ontario Statutory Accident Benefits Schedule (the “SABS”) provides a variety of benefits to individuals who are injured as a result of a motor vehicle accident.
In order to access those benefits, you are required to complete an application for accident benefits- this form is also referred to as OCF-1.
When you are involved in an accident, you are required to report the accident to your insurance company within 7 days. The insurance company will forward the OCF-1 to you (or you may pull it up on line, available here). The form must be completed and submitted to the insurance company within 30 days of your receipt.
The OCF-1 is crucial part of your claims process. It is very important that you complete the form accurately and truthfully. Failing to do so could result in your claim being denied and/or constitute a punishable offence under the Insurance Act.
The OCF-1 is a 7 page form and its completion is a time consuming process. Take the time needed to complete the form properly.
